County Model Programs - Administrator Application
2008 Model Program Application Packet (.doc, 196 KB)
Is for organizations wishing to administer one of the Model Cost-share Programs.
Deadline: 1st Friday of the month to receive Board Action at that month's meeting.
All model programs entered into agreement after January 1, 2008 are subject to the revised model program guidelines for that program.
Please note that all applications expire after 12-months, from the date stamped received in our office. If additional information is requested of an applicant and it is not received within that 12-month timeframe, then the application expires and the applicant must reapply.

Individual Application Components: Components requiring answers from the applicant are posted in Word (.doc) below, so that answers can be typed directly into the document. From the document, click on "File" in your browsers menu and select "Save As..." to save on your computer.
