Sample Evaluation Sheet:

Since the Technology Program is not a first-come-first-served program, a sample criteria/evaluation sheet for administrative use is available as an Adobe Portable Document File (.pdf) or a Microsoft Word (.DOC) file. Both documents are set up so that administrators can save the file and edit it on their own computers.
